Rather than manually creating all the sections and questions for your checklist, you do have the option of uploading a checklist.
When uploading a checklist, you have two options:
- You can create your own checklist from scratch by downloading the checklist template file.
- You can download one of our Sample Industry Checklists and use that as the starting point for your checklist.
Details to help you understand the elements in the checklist file are provided below.
To upload your checklist:
1) Create a new checklist. You will want to start from scratch in this case.
- The form input user interface will open when you create the checklist. If you are viewing your list of checklists, click on the checklist to open the form input screen.
2) In the form input interface, click on the settings icon, and Under IMPORT - click on Upload.
3) In the checklist import user interface click on the 'template file' to download a template if you have not yet prepared your file. Information for formatting the template file can be found below. If you have prepared your file, click Upload File, and browse for/select the file you wish to upload, then click on Continue.
- The file verification process will run, and confirm that the file is formatted correctly.
- If the file is formatted correctly, click on Submit.
If there are errors that need to be resolved before you can continue, they will be listed. Cancel the process, correct the errors in your file, and then upload it again.
The upload process may take a few moments, depending on how large your checklist is.
You will immediately be redirected to the form input interface when the upload is complete.
Understanding Checklist Files
Checklist files contain a header row as well as element rows. When uploaded, each row of the file (below the header) creates an element (i.e. a section or question) in the checklist.
You can modify or remove the element rows in the sample checklist or template file and you can add as many additional element rows as you would like, but the header row must remain as is.
The elements that can be created from a file include sections, simple check questions, and text input questions. If your checklist will have question types besides simple checks and/or text, you will have to manually add those to the checklist afterward.

| photo_required: | If you are adding a section, leave this cell empty. If you are creating a simple check question or a text input question and wish for the checker to supply a photo with the response to the question, enter 'y' in this cell. |
| disable_item: | If you are adding a section, leave this cell empty. If you are creating a simple check question or a text input question and wish for the element to be created but not readily available on the checklist enter 'y' in this cell. This is generally used for questions you will likely ask in the future, but not now. |
